Activity logs — overview

Per-area audit trail of every recorded action: who did what, when, against what.

Carerealm logs every meaningful action staff take against an area: diary entries created or edited, eMAR doses recorded, forms submitted, files uploaded, About sections changed, contacts edited, settings updated. The log is append-only — entries can't be edited or deleted.

Where the logs surface

  • Per- — open the area and click Activity Logs in the sidebar. Shows actions within that area only.

  • Realm-wide Activity Logs — for admins, Admin → Activity Logs shows actions across every area.

What each row contains

  • Date and time — when the action happened.

  • User — who did it.

  • Action area — which app the action was on (Diary, eMAR, Forms, etc.).

  • Action type — create, update, archive, delete, access, share, etc.

  • Action name — the specific event (e.g. "Diary entry submitted", "Medication administered").

  • Description — a one-line summary with relevant detail.

Who can read activity logs

Anyone with access to the area can read its activity log — visibility into "who did what" is part of the safeguarding culture. Realm-wide logs are admin-only.

Note — Activity logs are read-only. Even admins can't edit or delete a log entry. That's the point — the log is the audit trail.
