Contacts — overview

Per-area contact list: GP, family, professionals, anyone who supports them.

Each area has its own Contacts list — everyone associated with the supported person. Family, GP, social worker, district nurse, advocates, key workers, anyone external to your staff team.

What's on a contact

  • Name — required.

  • Company / organisation — optional. The GP surgery name, the local authority, the day-service provider.

  • Relationship — free text or a pick from a small list (e.g. Sister, GP, Speech & language therapist, Day-service key worker).

  • Email — one address.

  • Phone — one number.

  • Address — one postal address.

  • Notes — anything else worth knowing (best times to call, preferred mode of contact, language).

Note — Each contact carries a single phone number, email and address — if someone needs multiple (mobile + home, work + personal email), add them as separate contacts or include them in the Notes.

What's NOT in the contact form (yet)

  • Flags like "Emergency contact", "Next of kin", "Power of attorney".

  • Multiple phones / emails / addresses per contact.

  • CSV export.

Tip — For "this is who to call if X" guidance, use the contact's Notes field or an About section on the area so the same information is in the part of the record people read during incidents.
