A Yes/No question in Carerealm isn't just a switch. Below every yes/no toggle there's always an additional textarea: Additional information for "[question name]" (optional). Use it to explain the answer.
When to use the comment box
"Yes, but" — yes answers with context ("Yes — but only after I prompted twice").
"No, because" — refusals or omissions with a clinical reason ("No — supported person was asleep").
Observations — "Did they eat breakfast? Yes — finished two slices of toast and a tea."
Why it matters
Yes/no answers without context aren't very useful for inspections or handovers. The comment box turns a binary into a story — and inspectors notice when carers consistently leave it blank.
Note — The comment textarea is optional — yes/no questions don't require it even if the question itself is marked required. Just the switch needs an answer.
