Medication incidents

Record medication-incident events in eMAR (separate from the Forms-based incident system).

Medication-specific incidents are recorded directly in eMAR rather than in the generic Forms incident system — that's where the clinical context lives.

Incident types

  • wrong_dose — the wrong amount was given.

  • wrong_time — given at the wrong time of day.

  • wrong_resident — given to the wrong person.

  • wrong_medication — wrong medication picked.

  • omission — a dose was missed entirely.

  • near_miss — a potential error caught before administration.

  • adverse_reaction — the supported person had a reaction.

  • other — anything not covered above.

Severity

Each incident is graded by severity:

  • Minor — no harm, recorded for trend analysis.

  • Moderate — discomfort or minor harm.

  • Major — significant harm or hospital attendance.

  • Critical — life-threatening or fatal.

Safeguarding flag

Each incident can be flagged for safeguarding. Flagged incidents surface separately for safeguarding leads and may trigger workflows.

Recording an incident

  1. Open eMAR → More → Incidents.

  2. Click New incident.

  3. Pick the type, severity, and (if applicable) the medication.

  4. Describe what happened — facts, not opinions.

  5. Tick the safeguarding flag if relevant.

  6. Save.
