First — don't panic
A wrong recording isn't the end of the world if you correct it cleanly. Carerealm's audit trail keeps the original AND the correction, so honest mistakes show as mistakes (and that's appropriate).
If you recorded against the wrong medication
Add a correcting recording on the right medication with the correct outcome.
In the original (wrong) recording's notes, add a clarifying note explaining the mix-up.
Optionally log a Medication incident of type
wrong_medicationso trend reporting picks it up.
If you recorded the wrong outcome
Outcomes can't be silently changed. The right path depends on what you originally recorded vs what actually happened:
You recorded Given but the supported person actually refused — log a follow-up note, contact the nurse in charge.
You recorded Refused but they did take it — usually a non-event clinically. Add a note for clarity.
You recorded the wrong time — see retrospective recording.
If you recorded against the wrong supported person
This is a safeguarding event. Stop, escalate to the nurse in charge, and log a wrong_resident incident. The recording stays in the audit trail; the incident captures the safeguarding response.
Heads up — Never try to "delete" or silently rewrite a recording. The recording itself is the medico-legal record. The corrective flows exist precisely so honest mistakes are documented as mistakes — that's what an inspector wants to see.
