Sharing a template

Three modes: realm-wide (default), specific areas, or public URL.

A template's share mode controls which areas can run it (and whether the public can submit it without a Carerealm account). Two surfaces let you set it.

Surface 1 — inline on the builder

Open the template in the FormBuilder. The Form Sharing card has two toggles:

  • Make form public — when on, generates a unique URL anyone can use to submit, without signing in.

  • Share form with specific {{areas}} — when on, reveals an areas multi-select. When off, the template is shared with ALL areas in the realm (default).

Surface 2 — Sharing preferences dialog

From the admin shared-forms list → menu → Sharing preferences. A modal with three radio options:

  • Shared with all {{areas}} (default).

  • Shared with specific {{areas}} — reveals an areas picker.

  • Public form — generates a public URL.

Changing mode later

Changes apply on the next page load. Past submissions are unaffected — they stay where they were submitted.

Heads up — Switching from realm-wide to specific areas restricts new submissions. areas that lose access still see their past submissions but can't start new ones until they're re-added.

Public submissions

Public forms accept submissions from anyone with the URL. They land in the realm's shared submissions pool. Best for: external referrals, family feedback forms, public job applications. Don't make a clinical assessment public — it has no spam protection.
