Forms — overview

Templates, queued submissions, and completed history — all on one page with three tabs.

Forms in Carerealm are how a service captures structured records: assessments, audits, daily checks, incident reports. Inside an area, the Forms app opens to a single page with three tabs.

What's in it

Templates

Every form template available in this area — both area-specific and realm-wide "Shared Form" templates. Click "Complete form" on any row to start a submission.

Queued

Forms that have been queued for specific users to complete (with deadlines, reminders, recurrence). Anyone in the area can see the list; only assignees can complete.

Completed

Past submissions, filterable by submitter, form template, and date range.

Two layers

  • Templates — built by realm admins at Admin → Forms. Reusable across areas.

  • Submissions — instances of a template, filled in inside an area by a staff member.

Quick paths for a new carer

  • "I need to do my daily check" — Templates tab → find the template → click Complete form. See Filling in a form.

  • "Someone said they queued a form for me" — Queued tab → toggle Assigned to me → find the row → click Complete. Or look for the amber Due in... pip on a Templates row.

  • "I need to see what was submitted last week" — Completed tab → set the date filter.

Heads up — The dashed-underline form name on a Templates row goes to Manage details (a management page), NOT the completion flow. To start a submission, use the small Complete form button on the right of the row.
