Building a form template

The FormBuilder — adding elements, the per-element editor slide-over, conditional logic, save flow.

Templates are built at Admin → FormsAdd new template. The builder is a single scrolling column with a slide-over for each element.

Builder layout

  • Back link (top-left) — "All templates" or "All shared forms" depending on scope.

  • Inline-editable name + description — click either to edit. Both have a dashed underline as a hint.

  • Element count + last-saved — "N elements · last saved Xm ago".

  • Add new element button (top-right, primary).

  • Element list — drag-handle on each row, reorderable.

  • Sticky bottom bar — "There are unsaved changes…" with Save changes / Cancel, appears only when something has changed.

Element types (18 total)

  • Layout-only — Section heading, Section text block (rich-text), Picture (embedded image with no input).

  • Text inputs — Short text, Long text, Formatted text (rich-text editor).

  • Pickers — Single choice, Multi choice, Yes or no (with auto-comment box), Range slider (1-10 / mood).

  • Dates and times — Date picker, Time picker (with optional range).

  • Interactive — Body-map (click body diagram), Signature pad.

  • Uploads — Single image, Multi image, Single document, Multiple files.

Note — There is no dedicated numeric input type. For numbers, use Short text. There's no min/max length validation either — only the Is required toggle.

Per-element editor

Click any element row to open the editor slide-over (right side, ~half screen). Fields:

  • Element type — dropdown to change type after creation.

  • Title (or "Content" for text-block) — required.

  • Description — optional helper text below the question.

  • Type-specific fields — placeholder, available choices (for choice/multichoice), default value, time range toggle, range min/max, etc.

  • Is required toggle.

  • Show only when — conditional logic. Pick another question, an operator, and (if needed) a value. See Conditional questions.

  • Help information — extra hint shown to the user.

  • Remove element — outline button, turns red on hover.

Saving and publishing

There is no draft state — once you save changes, the template is live. Newly-created templates have no elements so they're effectively invisible until questions exist.

Heads up — There is no built-in preview mode in the builder. To test the assignee experience, save the template and open Complete form yourself from a test area. There's no way to preview without saving.
