Care plans and About sections per individual
About Me
Every supported person gets their own area with About sections, folders, photos, policies and read-tracking, organised the way your service operates.

Person-centred by design
One area, one person: everything that matters about them
Every supported person in Carerealm gets their own area. Inside that area live their diary, eMAR, forms, files, and their About Me sections. About Me is where the things that don't fit into a structured form go: who this person is, what they like, who matters to them, what good support looks like for them.
Sections are organised in folders, support text + photos, and carry their own revision history. Staff with access can see the latest version; admins can audit what changed and when.
- Per-area sections.
- Each section belongs to one supported person. No cross-talk between areas.
- Folder organisation.
- Group sections however you want: 'Communication', 'Daily routines', 'Health passport', 'Important people'.
- Photos in sections.
- Upload photos directly into a section, for visual identity, communication aids, or signposting around the home.

Know who's read what
Read tracking for policies and About sections
When an admin marks a section as a policy that staff need to read, Carerealm tracks who has actually opened and acknowledged it. A real audit trail for "has everyone seen the new behaviour-support plan?" that doesn't depend on chasing email replies.
- Read receipts.
- See, per user, when a section was last read. Unread acknowledgements surface in the dashboard.
- Revision history.
- Every edit is stored as a new revision. Reset read-status when content changes substantially.
Use your words
Your service, your terminology
Carerealm doesn't force a single label on the people you support. When you set up your realm you choose the word you use: Supported person, Client, Resident, Service user, Tenant, Customer, or Area. The label updates across the whole platform: navigation, button labels, page titles. You can change it later without breaking your data.
- Adapts to your sector.
- Supported living teams say 'supported person' or 'tenant'. Residential homes say 'resident'. Family carers may use the person's name in chat. Carerealm fits the language you already use.

What About Me isn't
About Me isn't a structured care-plan template builder. It deliberately gives you a flexible canvas (sections, folders, photos and read-tracking) rather than a fixed shape, because person-centred information rarely fits a template gracefully. If you want pre-built care-plan structures with required fields, the Forms module is the place to build those. Pair them with About Me for the narrative context.
